Outreach – Community Integration & Social Inclusion  

Our Outreach service provides flexible, person-centred support designed to enable individuals to live fulfilling lives within their communities. We focus on building confidence, independence and meaningful connections, ensuring that those we support are not only present in their communities but actively included and engaged.

We move beyond traditional support models by focusing on genuine inclusion, ensuring individuals are active participants in their communities, not passive recipients of care. We work alongside individuals, families, and professionals to create structured support plans that reflect personal goals, strengths, and aspirations

Delivered through tailored 1:1 support and community-based interventions, our outreach model is responsive to individual needs and promotes progression towards greater autonomy.  

Our support includes:

Community Engagement and Participation:​ Enabling access to local activities, education, volunteering, and employment opportunities, supporting individuals to build social networks and develop a sense of belonging.

 Social Skills and Relationship Building: Supporting communication, confidence and interpersonal skills to help individuals form and maintain positive relationships and reduce social isolation.

Independence and Life Skills Development:​ Practical support with daily living skills, including travel training, managing appointments, shopping, and navigating community resources. 

 

Emotional Wellbeing and Confidence Building: Providing consistent support to promote self-esteem, emotional regulation, and resilience, enabling individuals to engage more confidently in everyday life. 

Access to Services and Advocacy: Supporting individuals to understand and engage with health, education, and social care services, while promoting self-advocacy and informed decision-making
