Our transition packages are designed to deliver measurable outcomes, including increased independence, improved wellbeing, successful access to education or employment, and sustained tenancies
Support Includes:
Independent Living & Tenancy Sustainment: Development of essential life skills such as budgeting, cooking, self-care, and household management. We provide guidance on housing applications, understanding tenancy agreements, and maintaining accommodation, alongside travel training and community safety awareness.
Community Integration & Social Development: Support to access community activities, volunteering opportunities, and social networks, with a focus on developing communication skills, building relationships, and reducing social isolation.
Emotional Wellbeing & Mentoring: Dedicated mentoring support to build confidence, resilience, and self-awareness. This includes emotional regulation strategies, peer support opportunities, and structured sessions to promote long-term wellbeing.
Financial Independence & Benefits Support: Practical support with applications for benefits such as Personal Independence Payment (PIP) and Universal Credit, alongside budgeting and financial literacy to promote stability and reduce anxiety around income management
