Crisis & Stabilisation Support  

​​Our Crisis and Stabilisation service is outcome-focused, aiming to reduce risk, maintain placements, improve emotional wellbeing, and support individuals to remain safely within their communities. By delivering responsive, coordinated, and person-centred interventions, we help to prevent escalation and create lasting stability.

Our model bridges the gap between community-based support and acute services, delivering a credible and effective alternative to hospital admission while reducing system pressures and improving outcomes.

We work in close partnership with individuals, families, social care teams, and health professionals to ensure a joined-up response that is both responsive and sustainable. 

Our support includes:

Rapid Response Intervention:​ Flexible and responsive support delivered at short notice to stabilise situations of heightened risk. This includes increased staffing, outreach support, and immediate risk management to ensure safety and continuity of care.

 Placement Stabilisation: Intensive, short-term support to prevent placement breakdowns. We work alongside existing providers, families and professionals to strengthen care arrangements, reduce stressors, and maintain individuals within their current home or community setting.

Crisis Prevention and De-escalation:​ Implementation of informed approaches and low-arousal techniques, to reduce triggers and prevent escalation. Staff are trained to respond appropriately to needs, promoting calm and consistency during periods of distress. 

 Multi-Agency Collaboration: Close working with Local Authorities, community teams, GPs, and other professionals to ensure consistent communication, shared risk management, and aligned care planning. 

Individualised Crisis Planning: Development and implementation of personalised crisis and contingency plans, co-produced with the individual and their support network, to ensure clarity and preparedness in managing future risks.

Short-Term Intensive Support with Planned Step-Down: Our interventions are designed to stabilise situations effectively, followed by a gradual reduction in support as risks decrease, ensuring sustainable outcomes and a return to baseline or improved functioning.
